Report to Child Protective Services yourself or anonymously. It's the responsibility of adults to report, not a 4 year old. Praying.
You need to tell the authorities urgently who can examine him medically. Lord do something in this situation and protect this child. Move the mother to act quickly and wisely. Protect her and her son.
